Items with * are found in art and craft stores. An online link is included for each, so you can see what it looks like. However, you can probably get it cheaper, if you have a coupon for a local store.
*Artist's canvas with staples on the back
*Gloss gel medium (small amount.) Look for this near the acrylic artist paints. Online example
Foam brush, about 1" wide
Electrical tape, black. Get the cheapest you can find at the hardware store.
Chalk pencil, or washout pencil
Fabric scraps in the color of your choice. Stick with one color and gather fabrics that are very light to very dark. You'll need more dark than light.
Painting the background is very effective, but optional. These three were done that way: One, Two, Three. However, the background in this one was done entirely with scraps. If you'd like to paint the background of your collage, you'll need some additional supplies:
Scraps of white-on-white fabrics. These white fabrics have subtle designs of white that's sort of plastic-like. At least five such fabrics are recommended.
*Water soluble paints or crayons: watercolor, acrylic, water soluble wax pastels, etc. Online example
*Fixative spray for pastel and charcoal artwork. Online example
